New York State DMV Forms

The New York State DMV provides a wide range of forms for vehicle-related services, including driver licenses, vehicle registration, and sales tax certificate of authority. These forms can be found online and can be downloaded and printed for free. Some of the most commonly used forms are listed below. Part-1, Part-2.

  • MV-103 - Odometer and Damage Disclosure Statement: For use by a dealer or private owner where the title or dealer's certificate of sale does not have an odometer/damage disclosure statement section required by law.
  • MV-104 - Report of Motor Vehicle Accident: Required to be filed with DMV by all drivers involved in reportable motor vehicle accidents. See Motorist Accident Reports.
  • MV-1046 - TEENS (Teen Electronic Event Notification Service) Program: This form outlines the requirements and instructions to enroll in TEENS (Teen Electronic Event Notification Service).
  • MV-104C - Bicycle Accident Report: Required to be filed by a bicyclist when a bike accident involving death or serious physical injury occurs on a public highway, but DOES NOT involve a motor vehicle. Use form MV-104 to report accidents involving motor vehicles.
  • MV-104COV - Police Accident Report Cover Sheet: Cover Sheet for Police Accident Report forms MV-104A and MV-104AN. This cover sheet was formerly form MV-104AC.
  • MV-104F - Accident Report for School Vehicles Transporting Pupils/Teacher/Supervisors: Report completed for a school vehicle transporting students, teachers and/or supervisors involved in a motor vehicle accident.
  • MV-104F.1 - Accident Report for School Vehicles Transporting Pupils/Teacher/Supervisors (Continuation Sheet): Continuation sheet for additional information for form MV-104F.
  • MV-1056 - TEENS (Teen Electronic Event Notification Service) Program FAQs: Frequently asked questions relating to TEENS (Teen Electronic Event Notification Service).
  • MV-109 - Information and Certification for Vanpool Plates: Use to request Vanpool Registration
  • MV-114 - Information for Pick-up Truck Owners: Information sheet on obtaining passenger plates for pick-up trucks in order to use restricted highways such as parkways.
  • "MV-14 - Supplement to Driver's Manual for Tow Truck Driver's Endorsement: Use this manual to study for the written examination to add a tow truck driver endorsement on your driver license.

  • MV-1441.3 - Certification of Lost License, Permit or Plates: Complete this form if you are required to surrender your NYS driver license, learner permit or vehicle plates because of a suspension or revocation and the document or the vehicle plates are lost.

  • MV-15C - Request For Driving Record Information: As part of the Driver's Privacy Protection Act, use this form to request driving record information.
  • MV-15D - Motor Vehicle Record Search Account Application: Request for DMV records search account for government, commercial, and not-for-profit organizations, volunteer fire companies, and volunteer ambulance services..
  • MV-15DPPA - Federal Driver's Privacy Protection Act: Text of the Federal law controlling access to personal information on driver records and other DMV records. See information on the Driver Privacy Protection Act (DPPA).
  • MV-15GC - General Consent for Release of Personal Information: Used by a motorist to give consent for another person to obtain the motorist's personal information from DMV records. Usually filed with form MV-15. Must be notarized. See information on the Federal Driver's Privacy Protection Act.
  • MV-15PREED - Application for Access to DMV's Portal to Registration Electronic Enforcement & Display (PREED): Use this application to request access to DMV's Portal to Registration Electronic Enforcement & Display (PREED).
  • MV-15TKT - COMPASS Display Service Ticket View Application: For Tickets in COMPASS (a COMPASS search account is a prerequisite)
  • MV-1940 - Certification for Local Transit Service: The application required along with DOT authorization (form RA-1C) when requesting franchise bus registration.
  • MV-197 - Exempt Vehicle Certificate: For buses and ambulances that are not operated for-hire so vehicles are exempt from for-hire insurance, and registration fees in the case of some not-for-hire ambulances.
  • MV-198C - Request for Copy of an Accident Report: To request a copy of a motor vehicle accident report filed with the DMV by the police or a motorist. Provide as much information as possible about the accident. To request reports for more than one accident, you must complete a separate form for each accident. The DMV web site also has an online transaction to order and access accident reports.
  • MV-1E - Statement to Register a Vehicle Under Article 38: Completed by a disabled US veteran who has obtained a vehicle under Title 38 Chapter 39 of the US Code and is exempt from registration fees.
  • MV-1G - Statement to Renew Registration Under Article 38: Completed by a disabled US veteran renewing the registration on a vehicle obtained under Title 38 Chapter 39 of the US Code.
  • MV-1H - Statement to Register a Vehicle with Former POW or Congressional Medal of Honor Plates: Use to register a vehicle with Former POW or Congressional Medal of Honor Plates.
  • MV-1W - Withdrawal of Consent: Used by a parent or guardian to withdraw consent for a driver license or learner permit issued to a driver under age 18. When consent is withdrawn, the driver license or permit is cancelled. Consent is granted on form MV-44 or MV-44S.
  • MV-20 - Permanent Fleet Registration Program Guide: Information for owners of fleets of 200 or more vehicles that qualify for the Permanent Fleet Registration Program. Read the Overview on Page 1 for more information. Use form MV-82PFR (Permanent Fleet Registration Application) to apply.
  • MV-2001 - Claim and Release Form: Form used when filing a "small claim" against the DMV. See form MV-2001.1 for instructions.

  • MV-202C - Commercial Vehicle Registration Fee Schedule and Use Tax Chart: Also see Registration Fees and Use Taxes for Commercial Vehicles for updates.

  • MV-215 - Request for Refund: Use to apply for non-automated refunds of DMV registration and license fees authorized by law. DO NOT use for automated refund requests when surrendering plates at a DMV Office.
  • MV-232 - Address Change: Use to send a change of address for your regular or enhanced driver license, non-driver photo ID card and vehicle registrations. You can also order a new document with your new address. You can change your address online immediately.
  • MV-253G - Request for Business Amendment / Duplicate Certificate: Application to change or replace a business certificate for a dealer, inspection station, repair shop or other business regulated by DMV's Division of Vehicle Safety Services. When the facility reports a change of location, form VS-19 is also required. NOTE: See the important information about the recent change to DMV regulations for inspection station license applicants.
  • MV-260F - Certificate of Farm Vehicle Use: Must be completed when a registrant applies for a Farm Vehicle registration. Give Part 1 to the DMV office; keep part 2 with the vehicle registration.
  • MV-262 - Certification of Supervised Driving: Every driver license applicant who is 16 or 17 years old at the time of the road test, and who has a junior learner permit, must present this certification to the DMV license examiner at the time of the road test. The certification must be signed by a parent or guardian.
  • mv-263 - Online Permit Test Parent/Guardian Certification: Use to certify that the Online Permit Test was supervised by a parent or guardian for applicants under 18 years old.
  • MV-264 - Online Permit Test Attestation for Applicants 18 Years of Age and Older: Use to confirm that the Online Permit Test was completed without help from others.
  • MV-272 - Vehicle Identification Number for Homemade Trailer: Use to request a VIN for a homemade trailer weighing 999 lbs. or less.
  • MV-274 - Trailer-Type Vehicles Not Requiring Registration: List of trailer-type vehicles that are not required to be registered as trailers in NYS. Form explains how to request information about whether similar vehicles must be registered.
  • MV-277 - Pre-licensing Course Instructor's Manual: Curriculum for the driver license pre-licensing course.
  • MV-277.1 - Pre-licensing Course Student's Manual: Student's manual for the driver license pre-licensing course.
  • MV-278.5N - Roster and Control (Pre-licensing Course): Completed by driving schools to record information about MV-278 certificates issued to students who complete the pre-licensing course.
  • MV-278.6 - Authorized Signature List (Pre-licensing Course): Completed by driving schools to provide names and signatures of individuals authorized to request and issue course completion certificates for the pre-licensing course.
  • MV-278.7 - Authorization and Approval for Pre-licensing Course (Secondary Schools and Colleges): Used by secondary schools and colleges to request approval to issue pre-licensing course certificates (MV-278) in conjunction with their driver education programs, to teach the pre-licensing course or to notify the DMV of any changes related to the pre-licensing course.
  • MV-278.8 CDS - Pre-Licensing Course Completion Certificate Order Form for Commercial Pre-Licensing Course Providers: Used by commercial driving schools to order MV-278 pre-licensing course completion certificate books.
  • MV-278.8 SSC - Pre-Licensing Course Completion Certificate Order Form for Secondary Schools And Colleges: Used by secondary schools and colleges (high schools, colleges and BOCES) to order MV-278 pre-licensing course completion certificate books.

  • MV-279 - Request for Premises Check: To request pre-licensing classroom approval by the DMV.
  • MV-285R - Student MV-285 Roster: Completed by high schools, colleges and BOCES to record information about MV-285 certificates issued to students who complete a driver education program.
  • MV-289 - Certification for Taxi Plate: Certification so a taxi owner to be issued a taxi plate.
  • MV-299.1 - Section 394 Drivers' Schools, NYS Vehicle and Traffic Law: Text of Section 394 of the NYS Vehicle and Traffic Law governing driving schools and driving instructors.
  • MV-299.4 - Receipts and Contracts: Instructions for customer receipts and contracts, fill-in receipts for pre-numbering and printing that follow the guidelines driving schools are required to use, and a sample of the customer contract that driving schools are required to use.
  • MV-2CDL - Requirements for Commercial Drivers: Required for all commercial driver license (CDL) renewals.
  • "MV-327 - Notice of Repossession of Motor Vehicle or Motorcycle: Use to notify DMV, as required by law, when a vehicle is repossessed. Three copies of the form are included. Present all three copies to a DMV office and the copies marked for the Repossessor and the Owner will be returned to you. See also form MV-950.

  • MV-349 - Transfer of Vehicle Registered in Name of Deceased Person: Use to transfer the vehicle of a deceased person when there is no will, estate, surviving spouse or surviving minor child, and the vehicle is worth $25,000 or less. For information see Deceased Vehicle Owner.
  • MV-349.1 - Affidavit for Transfer of Motor Vehicle: Use to transfer a vehicle owned by a deceased person to a surviving spouse or to a child or children under age 21, if the vehicle is worth $25,000 or less. For information see Deceased Vehicle Owner.
  • MV-35 - Statement of Vehicle Owner Not Possessing a Valid Title: Use to transfer a vehicle at least eight model years old and worth less then $1250 to a vehicle dismantler, itinerant vehicle collector or certified scrap processor when the owner does not have the title.

  • MV-37 - Statement of Abandoned Vehicle: Use to transfer a vehicle at least ten model years old and worth less then $1250 from a property owner to a vehicle dismantler or itinerant vehicle collector when the vehicle has been abandoned for at least one month.
  • MV-372 - Application for a License to Operate a Private Service Bureau or Open a Branch Office: Application for the license required of any person, form, association or corporation engaged in the business of assisting for hire in securing licenses to drive motor vehicles, registrations, or titles of motor vehicles.
  • MV-372R - Application for Private Service Bureau License Renewal: Application used by a Private Service Bureau to apply for renewal of its PSB license.
  • MV-3W.3 - Registrant Certification of Heavyweight Vehicle: Required to be filed when registering trucks that weight 44,801 pounds or more in the Agricultural, Commercial, Commercial Household Carrier and Tractor registration classes.
  • MV-415 - Request for the Development of Custom Plates: Information package and request for the development of custom plates.
  • MV-44 - Application for Permit, Driver License or Non-Driver ID Card: Use to apply for a learner permit, driver license or non-driver ID. Also use to renew your license or non-driver ID.
  • MV-44.5 - Federal Requirements for Commercial Driver License (CDL) Applicants: Information for applicants for original or renewal NYS commercial driver licenses.

  • MV-44CDL - Commercial Driver License Class A Young Adult Training Program Certification of 300 Hours of Supervised Behind-the-Wheel Training: This form is associated with the CDL A Young Adult Training Program. Applicants for a CDL Class A License who are 18, 19 or 20 years old must bring this completed certificate to the DMV office and give it to a Motor Vehicle Representative at the time of license issuance or amendment.
  • MV-44NC - Application for Name Change ONLY on Standard Permit, Driver License or Non-Driver ID Card: Use this form to apply for a name change only on an existing Standard learner permit, driver license or non-driver ID.
  • MV-44NYR - Certification of Residence: Used with an application for an enhanced driver license (EDL), enhanced learner permit or enhanced non-driver photo ID card (ENDID).
  • "MV-45 - Statement of Identity by Parent/Guardian: Completed by parent of child under age 21 to provide 4 points of proof of ID for child applying for permit/license, registration/title. The form MV-45 can also be used to provide 1 proof of residence for a child who applies for an enhanced document. This form MUST be signed in the presence of a DMV representative.

  • MV-454 - Affirmation for Renewal of Registration for Absentee Owner: This form is to be filed with a registration renewal when the registrant is in the military, cannot be reached to sign the renewal application, and the vehicle is being used by a member of the registrant's family with his or her permission.
  • MV-456 - Certification of Disposition of Vehicle on Behalf of Absentee Owner: Completed by a person who has been given permission to sell a vehicle owned by a member of the armed services. Owner's title or transferable registration is also required to transfer.
  • MV-45A - Statement Of Identity - For Applicants Represented by Government or Government-Approved Facilities: Identity and/or residence affidavit used by an applicant who is mentally or physically challenged when the applicant cannot provide sufficient proof of identity and/or residence and is being represented by a government or government-approved facility.
  • MV-45B - Statement Of Identity - For Applicants Who Can be Considered a Disenfranchised, Homeless Youth: Identity affidavit used by an applicant who can be considered a disenfranchised, homeless youth, and is being represented by a government or government-approved facility.

  • MV-463 - Application for Dealer Plate Issuance Program: For qualified auto and ATV dealers who wish to issue registration plates to customers.

  • MV-500H - Important Information About Learner's Permits: Restrictions for any person who has a Class DJ or MJ Learner Permit.
  • MV-51 - Affidavit of Sale or Transfer: Use to certify ownership and to transfer a non-titled vehicle which was never registered by the current owner. May not be used by auto dealers and must be accompanied by other proofs of ownership.
  • MV-519 - Information Concerning Oversized/Overweight Vehicles: Provides information on maximum vehicle weights and dimensions in NYS and how to obtain oversize/overweight permits.
  • MV-51B - Statement of Ownership for Non-Titled Vehicles, Boats, Snowmobiles and All-Terrain Vehicles: Use to certify ownership of a non-titled vehicle, vessel, snowmobile or all-terrain vehicle and to certify why proof of ownership is not available. Must be accompanied by other documents listed on the form.
  • MV-520 - Application for Original Driving School License: Application for original license for a driving school. Revised June 2020.
  • MV-521 - Application to Amend a Driving School License: Application to amend an existing commercial driving school license.
  • MV-521.1 - Personal History: Required for all owners, partners, corporate officers, managers, agents, employees and major stockholders of a driving school or private service bureau.
  • MV-522 - Driving School Renewal Application: Used to renew a driving school license.
  • MV-522.1 - Access to DMV Internet Road Test Scheduling System: Application for driving schools to use the DMV online road test scheduling system.
  • MV-523 - Application for Driving School Instructor Certificate: Application for certification as a driving school instructor. Revised March 2020.
  • MV-526 - Driving School Instructor Termination Notice: Use this form to notify the Department of Motor Vehicles about the termination of a driving school instructor as required by DMV Commissioner's Regulation 76.15(f). Provide a copy of this termination notice to the instructor.
  • MV-527 - List of Driving School Vehicles: Required to be filed by driving school applicants, listing all vehicles used by for instruction or road tests.
  • MV-529 - Vehicle Equipment for Passenger Vehicles: List of required equipment for passenger vehicles.
  • MV-529A - Equipment Required for Commercial Vehicles: List of required equipment for commercial vehicles.
  • MV-529B - Equipment Required For Motorcycles - Including Limited Use Motorcycles: List of required equipment for motorcycles and limited use motorcycles (mopeds).
  • MV-529C - Equipment Required for Trailers: List of equipment required for trailers.
  • MV-52SN - Snowmobile Dealer Registration Instructions: Instructions on how to register as a snowmobile dealer and how to process customer snowmobile registrations.
  • MV-58A - Certificate of Employment: Use to certify employment of junior driver. Completed by the employer of a junior driver. The junior driver carries the form when permitted to drive to and from work without a parent, guardian, driver education teacher or driving school instructor.   • MV-619 - Vision Test Report: Completed by authorized vision test provider to certify a driver meets basic DMV vision standard (otherwise, use form MV-80L). May be used instead of taking vision test at a DMV office so a license can be renewed online or by mail.
  • MV-65 - Vehicle Escort Driver Application: Application for certificate to operate as an escort driver for oversize vehicles. Applicants must pass a written test based on the NYS Certified Escort Manual available from DMV Offices or Call Centers.
  • MV-653 - Certification of Eligibility for Government/Official Plates: Used only by NYS political subdivisions to certify eligibility for Government/Official plates.
  • MV-653EM - Certification of Eligibility for Emergency Management Plates: Certification of Eligibility for Emergency Management Plates
  • MV-653V - Volunteer Fire Company or Volunteer Ambulance Company Certification of Eligibility for Official Plates: Used only by volunteer fire companies or volunteer ambulance companies to certify eligibility for Official plates.

  • MV-663 - Certification for an Additional Set of License Plates for Persons With Disabilities: Use if a disabled individual already has plates for a person with disabilities and is requesting an additional set for a second vehicle registered to him/her and used by another disabled family member.   • MV-664.1 - Application for License Plates or Parking Permits for People with Severe Disabilities: Used by New York residents to apply for plates or a parking permit for people with severe disabilities. Requires a medical doctor's signature and certification. Plates are issued at state and county DMV offices, but permits are issued by local governments, not DMV. Filing a false application is a crime.
  • MV-664.2MP - Metered Parking Waiver Information: Information for a NYS resident with a severe disability to apply for a metered parking waiver.
  • MV-700 - Application for Sales Away from Premises: Completed by a car dealer who wishes to apply for a sale away from the premises of their registered location. This application must be received by DMV at least 20 days before the sale is to begin.
  • "MV-75 - Notification of Military Service: Use this form to notify NYS DMV that you are entering, or are in the military, and wish to have your driver status maintained. There is no fee to file this Notification.
  • This is a two-part form. Send Part 1 to DMV and keep Part 2 with your driver license.
  • Note: When you fill-in information in Part 1 of the online form, the fields in Part 2 are automatically filled in for you."
  • "MV-79 - Driver's Manual for the Safe Securement of Metal Coils and Other Cargo: Manual to study for the written test required for the metal coil (M) endorsement requirement for certain commercial driver licenses (CDLs).

  • MV-80 - Physician's Statement: Completed by physician treating a condition that was reported by a driver license applicant on application form MV-44.
  • MV-80L - Eye Test Report for Medical Review Unit: For people who cannot pass basic DMV vision screening and need to provide eye care practitioner certification of minimum vision requirements to the DMV.
  • MV-80S - Declaración del médico: Lo completa el médico que trata una afección que informó un solicitante de licencia de conducir en el formulario de aplicación MV-44.
  • MV-80U.1 - Physician's Statement for Medical Review Unit: Use this form, if requested by DMV, to provide medical documentation from you and your doctor regarding a loss of consciousness or a loss of body control.
  • MV-80W - Application for Tinted Window Exemption: Used to apply for a medical exemption from the restrictions on the amount of tint allowed in vehicle window glass. The applicant must also submit a physician's statement on the physician's letterhead about the specific medical condition.
  • MV-82 - Vehicle Registration/Title Application: Use to register vehicles, renew vehicle registration, amend or request duplicate vehicle registration. Be sure to download form MV-82.1, found below. You can renew your registration or order a replacement registration online.

  • MV-83SAL - Salvage Examination/Title Application: Used to apply for a salvage examination and title.
  • MV-83T - Statement of Partnership or Joint Ownership: Used in conjunction with a registration/title application when more than two persons are partners/joint owners of a vehicle.
  • MV-843 - Transfer of Ownership When the Vehicle Owner is Deceased: Complete this checklist in order to transfer ownership of a motor vehicle when the owner is deceased.
  • MV-900 - Notice of Lien: Filed by lienholder to record a lien on a vehicle title. A $5.00 fee must be paid by lienholder. Both lienholder and owner must sign form. If a title has already been issued, original title must be submitted with this form.
  • MV-900.1 - Notice of Lien - Charge Account Customer: Filed by a lienholder who has a charge account with the DMV to record a lien on a vehicle title. If you do not have a charge account, use form MV-900. A $5.00 is charged to the escrow account of the lienholder. Both lienholder and owner must sign form. If a title has already been issued, original title must be submitted with this form.

  • MV-905 - Self-Storage Owner/Operator Affirmation and Bill of Sale: Sworn statement by a self-storage business owner/operator concerning the sale of a vehicle or boat to obtain compensation for unpaid storage charges.
  • MV-907M - Disposition for Junk and Salvage Vehicles (Multiple Entry Form): To be used only by a registered vehicle dismantler or scrap processor to record the destruction of multiple vehicles.
  • MV-908 - Salvage Vehicle Waiver: This form is to be used by insurance companies to attest that a theft recovery vehicle meets the definition pursuant to Sec. 430 of the NY Vehicle & Traffic Law for a salvage vehicle waiver. The MV-908 is a standardized form that replaces insurance company affidavits previously prepared and submitted on company letterhead for this purpose.

  • MV-950 - Affirmation of Repossession and Bill of Sale: Required to be completed when a lender repossesses a vehicle, and used to transfer the vehicle to a new owner.
  • MV-994 - Bond Under Section 2105 (d) of the New York State Vehicle and Traffic Law: For use by an insurance company when a Surety Bond is posted by a person who does not have adequate proof of ownership for a vehicle and cannot obtain it. This form must be completed by the insurance company and signed by them (Attorney-in-Fact) and the owner (principal).

The New York State DMV offers a variety of forms for vehicle-related services, including driver licenses, vehicle registration, and sales tax certificate of authority.
