• AA-15 - TVB Ticket Management for Attorneys Application for Web Access: Used by an attorney who is licensed to practice law in New York State, and who represents clients at their Traffic Violations Bureau (TVB) hearings, to apply for an Internet access account to manage client traffic tickets and schedule TVB hearings online.
• AA-3.3 - Applications to Open Default Conviction: Use this form to request that the DMV reopen a Traffic Violations Bureau traffic ticket from New York City that resulted in a default judgment against you. You must include any documents that may be necessary to support your statements, and you must be willing to participate in a hearing. This form cannot be used in local courts or for tickets where a default judgment was not entered.
• "AA-33 - Traffic Violations Bureau Appeal Form: Use to file an appeal from a conviction and/or penalty imposed for non-criminal moving violations after a DMV Traffic Violations Bureau hearing in New York City only (see Traffic Tickets). For convictions or penalties by courts in other locations and courts, contact the court, not DMV, for appeal information.

• AA-33A - Appeal From DMV Hearing Determination: Use to file an appeal from a finding and/or penalty imposed after a DMV hearing regarding such matters as a fatal accident, chemical test refusal, or motor-vehicle related business regulated by DMV. Also used to appeal denial by DMV of driver license or business permit application. Cannot be used to appeal suspensions or revocations resulting from traffic violation convictions in a court or a DMV Traffic Violations Bureau.
• AA-33FI - Freedom of Information Appeal Form: Used to appeal a denial of access to records.
• AA-53.2 - Statement In Place of Personal Appearance: A Statement In Place of Personal Appearance - Unrepresented motorists who do not want to appear in person for a hearing, may appear by submitting a written statement instead. To appear by written statement, schedule a hearing and then complete and sign the Statement in Place of Personal Appearance form and mail it to the office where your hearing is scheduled to be held. At the hearing, the judge will hear testimony, read your statement into the record and consider any evidence presented. The judge will make a finding of “guilty” or “not guilty” and mail you a decision to the address provided in the Statement.
• AA-71 - Franchised Motor Vehicle Dealer Request for Adjudicatory Proceeding: Used by a franchised motor vehicle dealer to request that a grievance with the franchisor be heard before a DMV administrative law judge.

• DPR-102 - Impaired Driver Program Instructor Application: Used to apply to become a Impaired Driver Program instructor. A separate form DPR-102 must be completed for each instructor.

• A resume for the IDP Director must be included with the form DPR-103. The resume must include the name, address, telephone number, DMV Client ID Number from the driver license if applicable, the educational background and the employment background of the Director.

• DPR-104 - Impaired Driver Program Service Agreement: All Impaired Driver Programs must agree to the terms included in form DPR-104 in order to operate an IDP. The Service Agreement defines the responsibilities of the IDP to the motorist, to the program, and to the DMV. Form DPR-104 is required to be submitted with form DPR-103 and all original application materials. The applicant must also submit a form DPR-102 for each instructor.
• DPR-125 - Impaired Driver Program (IDP) Classroom Site Inspection Report: All Impaired Driver Programs must submit this form with IDP section completed for DMV to inspect the proposed classroom site.

• DS-103 - Report of Passing Stopped School Bus: Report of Passing Stopped School Bus. A motor carrier must complete this form.
• "DS-115 - Request for NYS Driving Privileges: Used by out-of-state drivers to request restoration of their New York State driving privileges following a revocation of privileges.
• This form cannot be used to request restoration of a NYS driver license or learner permit following a revocation. Revoked NYS drivers must apply for a new license using form MV-44."
• DS-19 - Article 19-A Motor Carrier Accident and Conviction Notification Program Application (Escrow Account & Driver's Privacy Protection Act Compliance): Article 19-A Motor Carrier Accident and Conviction Notification Program Application. New carriers applying for 19-A status must file using this form. This form includes both a Memorandum of Understanding - Driver's Privacy Protection Act and a Request for Article 19-A Accident & Conviction Notification Account.
