The Texas Health and Human Services (HHS) issues the Preadmission Screening and Resident Review (PASRR) Report of Specialized Services Non-Compliance to Consumer Rights and Services (CRS), Form 2009. This report is designed for Local Intellectual and Developmental Disability Authorities (LIDDAs) to document non-compliances with consumer rights and services in nursing facilities.

The form, PEN Form 2009 SG, is used to track complaints related to the failure to initiate or provide ongoing specialized services, including occupational therapy (OT), physical therapy (PT), speech therapy, and community-based waiver services. The report requires LIDDAs to submit information on each individual complaint, including the nursing facility name, address, and resident Medicaid number.

The purpose of this form is to ensure that LIDDAs are monitoring and reporting non-compliances with consumer rights and services in a timely manner. The report must be submitted by the 15th of every month for the previous month, and can be faxed to 512-438-2180 with attention to PASRR QM Staff. This form is an essential tool for promoting accountability and improving the quality of life for individuals with intellectual and developmental disabilities in Texas nursing facilities.
