Form DS-71. Affidavit of Identifying Witness

The Affidavit of Identifying Witness, represented by Form DS-71, is used in conjunction with Form DS-11, the Application for a U.S. Passport. This form is completed only by the identifying witness and serves to confirm the identity of the passport applicant.  OMB No. 1405-0088.

Usage Case: This form is required when a passport applicant is unable to establish their identity to the satisfaction of the authorized person accepting passport applications. The witness must complete this form, sign it as requested by the acceptance agent, and provide a photocopy of their identification, both front and back. The witness affirms, under penalty of perjury, their knowledge or belief that the passport applicant is a U.S. citizen or non-citizen national and that the statements provided are true and correct.

  8. Warning: A notice about the legal consequences of false statements or misuse of the passport.
  9. Privacy Act Statement: Explains the authorities, purpose, routine uses, and disclosure policies related to the collected information.
  10. Paperwork Reduction Act Statement: Informs applicants about the estimated time required to complete the form and where to address comments or concerns.

The form is used to ensure the identity of the passport applicant and to comply with the law regarding passport applications.
