AF Form 4391 - High-Risk Activities Worksheet

AF Form 4391 is used to document and assess the risks associated with high-risk activities in which personnel may participate. The purpose of this form is to ensure that personnel are aware of the hazards and injury potential of these activities and take necessary safety precautions.

Instructions:

  1. Complete the individual data information section with your name, age, unit/office symbol, duty phone, and list of high-risk activities you participate in.
  2. Provide the frequency of participation, previous experience, and hazards associated with each activity.
  3. Identify any specialized training required or completed for each activity.
  4. Specify the location/area where the activity will occur, including business name, address, and phone number.
  5. List any preconditions agreed upon during briefing, such as specific location, special equipment, or medical screening.
  6. The form must be signed and dated by the individual participating in the high-risk activity.

Authority: This form is authorized under 10 U.S.C., Chapter 40, 37 U.S.C., Chapter 9, EO 9397, November 1943.

Principle Purposes: This form is used for documentation of occupational safety, fire protection, environmental, and health training.

Routine Uses: This form will be used to create a record of training and to retrieve records of safety, fire protection, and health training.
