The Virginia Department of Motor Vehicles (DMV) provides various forms for different purposes, such as license and identification card applications, vehicle information requests, driver improvement courses, and hazmat endorsements.   5. CSMA 01 - Contact Information for Vital Statistics: This publication lists contacts to obtain a certified copy of a birth certificate.
  6. CSMA 14 - Customer Concern Report: A DMV customer uses this form to express concerns about groups outside of DMV facilities.
  7. CSMA 19 - In-Car Maneuvers Observation Record: This form is used by (1) learning drivers age 18 who hold a learner’s permit, (2) learning drivers age 19 and older who hold a learner’s permit but have not completed a Virginia approved driver training course, and (3) qualifying home-schooled students to verify the observation and completion of safe driving maneuvers before applying to take the road skills test at DMV.

  13. DIC551 - Online Driver Improvement Clinic - License Application and Agreement: Use this form to apply for or renew an online driver improvement clinic license.
  14. DIC 550 - Curriculum Vendor Agreement for Computer-Based Driving Improvement Training: Use this form to apply for a Curriculum Vendor Agreement for the Computer-Based Training Driver Improvement Clinic Program.
  15. DL 1P - Driver's License and Identification Card Application: Use this form to apply for a Virginia Driver's License or Identification Card.
  16. DL 2P - Commercial Driver's License (CDL) Application: Use this form to apply for a commercial driver's license or commercial learner's permit.
  17. DL 2PA - Supplemental Driver’s Licensing History (for CDL Applicants): A Commercial Driver License (CDL) applicant transferring to Virginia from out-of-state uses this form to record all driver's licenses issued in the past 10 years. Applicants for a CDL renewal or reissue complete this form only if a driver's license history check has not been previously performed.
  18. DL 5 - Identification Card Application for Minors Under Age 15: Minors under age 15 use this form to apply for a Virginia Identification Card.
  19. DL 7 - Exchanging a Driver's License from a Foreign Country: Use this form to request an exchange of a valid driver's license from certain foreign countries for a Virginia driver's license.
  20. DL 8 - Commercial Driver’s License (CDL) Self-Certification: Use this form to record and/or change a CDL driver's self-certification category for the driver's type of commercial motor vehicle operation as required by VA Code § 46.2-341.12(G).
  21. DL 10 - Driver Privilege Card Application: Non-US citizens may use this form to apply for a Driver Privilege Card or Identification Privilege Card.
  22. DL 11 - Virginia Veteran Military Service Certification: Virginia veterans include this form when applying for a veteran indicator on their Virginia license or identification card (ID)
  23. DL 13 - CDL Test Waiver Military Application: Use this form to request a waiver of the CDL Skills Test when applying for a CDL for vehicle class(es) representative of the commercial motor vehicle operated during military service.
  24. DL 18 - Cancellation of Minor's Driving Privilege: Use this form to allow a custodial parent or guardian of an unmarried or unemancipated, after the minor has been issued a driver's license, to request the minor's driving privilege be cancelled.
  25. DL 23 - Identification Privilege Card Application For Minors Under Age 15: Use this form to qualify for an identification privilege card for a minor.
  26. DL 55 - National Driver Register File Check - Employer Request: Employers use this form to request a National Driver Register (NDR) file check on a current or prospective employee.
  27. DL 56 - National Driver Register File Check - Individual Request: Use this form to request a National Driver Register (NDR) file check on yourself and obtain a copy of the record if one exists.
  28. DL 70 - Hazmat Endorsement Background Record Check: Use this form to apply for a hazardous materials endorsement (hazmat) background check. This formcollects information required by the Transportation Security Administration.
  29. DL 80 - Electronic Birth Certification Request – Not Born in Virginia: Customers born in a U.S. state (excluding Virginia) use this form to request an electronic certification of a birth if they do not have a paper birth certificate when applying for a Virginia Driver's License (DL) or Identification Card (ID).
  30. DL 81 - Electronic Birth Certification Request – Born in Virginia: Customers born in Virginia (from 1912 to the present) use this form to request a Virginia birth certificate.
  31. DL 82 - Virginia Vital Record Application - Marriage/Divorce/Death Certificate: Customers use this form to provide information for the Virginia Department of Health, Division of Vital Records (VDH) to certify/produce a Virginia death, marriage or divorce certificate.
  32. DL 120 - Entry Level Driver Training Self-certification Of Exemption: Use this form if you held a commercial driver's license (CDL) prior to February 7, 2022 and are exempt from the federal Entry Level Driver Training requirements under 49 CFR parts 308, 383 and 384 and Va. Code § 46.2-341.12.
  33. DL 121 - CDL Entry Level Driver Training (ELDT) Re-examination Course Completion Self-certification: This form may be used by commercial driver's license (CDL) applicants who failed the CDL general knowledge or road skills test three times prior to February 7, 2022 to self-certify to the completion of Entry Level Driver Training in order to satisfy the requirement to complete driver training prior to testing a fourth time.
  34. DL 145 - Traumatic Brain Injury Designation Application: Use this form to apply for a traumatic brain injury designation on your driver's license.
  35. DL 5 - Identification Card Application for Minors Under Age 15: Minors under age 15 use this form to apply for an identification card.
