TX HHS Form 5525-NATCEP. Request to Take the Competency Evaluation Program (CEP) Based on Completion of Approved Nurse Aide Training Program that is Currently Closed

The applicant must not be listed as unemployable on the Employee Misconduct Registry (EMR) or have a conviction of a criminal offense listed in the Texas Health and Safety Code §250.006.
