TX HHS Form 3246. Eligibility Determination Application for Early Termination of a Persons Obligation to Register

The TX HHS Form 3246, "Eligibility Determination Application for Early Termination of a Person's Obligation to Register," is used to determine eligibility for early termination of a person's obligation to register as a sex offender. This form should be completed by individuals who wish to terminate their registration requirements earlier than required.

The application requires providing information about the offender, including legal name and date of birth. Additionally, the applicant must provide attorney information if represented, reportable convictions or adjudications, and supporting documentation regarding the offense. The supporting documentation must include official documents showing the age of the victim at the time of the offense for certain types of convictions.

The application also requires a $50 administrative review fee, which is non-refundable. Completed applications with required fee should be mailed to CSOT at P.O. Box 149347, Austin, TX 78714-9347. Note that CSOT will not review an application unless it includes all required supporting documentation.
