TX HHS Form 1070. Offer of Texas Home Living Program

The Texas Home Living Program (TxHmL) waiver program services offer is provided by the Local Intellectual and Developmental Disability Authority (LIDDA), as notified by the Texas Health and Human Services Commission. This lifelong program aims to meet your changing needs, allowing you to choose any TxHmL provider in the state and transfer services at any time.

The TxHmL program offers a range of services, including professional therapies, day habilitation, employment assistance, behavioral support, respite, supported employment, nursing, community first choice (CFC) personal, minor home modifications, dental attendant/habilitation services, dental adaptive aids, and community support (transportation). These services are designed to supplement natural supports or other programs, not replace them.

You must respond to this offer within 30 calendar days by contacting the LIDDA at the provided address or phone number. Failure to respond may result in your name being removed from the interest list and the offer being formally withdrawn.
