TX HHS Form 1067. Offer of Home and Community-based Services (HCS) Program

The Texas Health and Human Services (HHS) has issued the Offer of Home and Community-based Services (HCS) Program Form 1067 to provide life-long support for individuals with intellectual and developmental disabilities. This program aims to supplement existing services, not replace them, and offers a range of services including professional therapies, residential assistance, behavior support, respite nursing, day habilitation, dental care, and community first choice (CFC) personal attendant/habilitation services.

The form is issued by the Local Intellectual and Developmental Disability Authority (LIDDA) and requires a response within 30 calendar days of receipt. Failure to respond may result in the offer being withdrawn, and if under 22 years old and living in a nursing facility or intermediate care facility for individuals with an intellectual disability or related conditions (ICF/IID), the individual's name will be placed back on the HCS interest list with a new registration date.

This notice provides important information about the HCS Program, including the requirement to respond to the LIDDA and the potential consequences of declining the offer. The form also includes instructions for reinstating one's name to the interest list if removed. For any questions or concerns, individuals can contact the LIDDA staff as shown on the form.
