OCFS-3128. Program Expenditure Report - Contracted Services & Stipends
Form OCFS-3128 is a Program Expenditure Report, used to detail the costs of contracted services and stipends for a particular program. It is used by New York State Office of Children and Family Services (OCFS) to report expenditures and to support cost allocations, for both contracted services and stipends.
The form requires the Program Director or designee to provide the program name, program ID, fiscal year, and expenditure period for the report. There are two sections in the form: contracted services and stipends. The contracted services section requires details of the vendor, description of the service, service start and end date, the amount paid, and a brief description of the service provided. The stipends section requires details of the employee or contractor, their job title, rate of pay, pay period, total hours worked, and amount paid.
Accurate completion of the form is important for OCFS to ensure that the services and stipends provided are in compliance with their contractual obligations and applicable regulations. If the form is not completed accurately and completely, it may result in inaccurate cost allocations and financial reporting. Incomplete or inaccurate forms may also lead to audit findings and/or corrective actions.
The form is typically used by Program Directors or their designees to report the costs of contracted services and stipends for their programs. This form provides an overview of the costs incurred by the program, and is used to track the costs associated with the contracted services and stipends. This form can also be used to identify any inconsistencies in the data, and to ensure that the expenditures are in compliance with the contractual obligations and applicable regulations.
In addition to the OCFS-3128 form, additional documents may be required to be completed when submitting the form, such as the vendor invoice or employee timesheet. There are also other forms that may be used for similar transactions or purposes, such as the OCFS-3129 form, which is used to report the costs of purchased services.
