In order to complete the form, the applicant must provide information about his or her identity, such as name, date and place of birth, and Social Security number. The applicant must also provide details of their relationship to the employee of the U.S. government abroad, including the employee’s name, dates of employment, and country of employment.
Additional documents may be required when submitting the form, such as the applicant’s passport, evidence of his or her relationship to the employee of the U.S. government abroad, evidence of the employee’s employment abroad, and a certified translation of any foreign language documents.
Form DS-1884 must be submitted to the United States Citizenship and Immigration Services in order for the special immigrant classification to be processed.
An alternative to Form DS-1884 is Form N-648, Medical Certification for Disability Exceptions. This form is used by the applicant to prove that he or she is disabled and unable to take the English language or civics tests required to become a U.S. citizen.
