CBP Form 3078: Application for identification card

CBP Form 3078, also known as the Application for Identification Card, is a form used to apply for an identification card that will give the recipient access to CBP security areas. This form is typically filled out by those whose jobs involve receiving, handling, or transporting imported goods that are yet to be released from CBP custody, such as brokers or employees, warehouse officers, foreign trade zone employees, and license cartmen. The form itself is composed of different sections that require biographical information of the applicant.

The primary purpose of Form 3078 is to verify the identity of an applicant and to allow CBP to conduct background checks in order to determine if they meet the criteria stated by the CBP to be eligible for the identification card.
