TX HHS Form 1487. Designation of Representative
The Texas Health and Human Services (HHS) Form 1487, Designation of Representative, allows individuals to designate a representative to act on their behalf for the purpose of rehabilitation services. This form is used to authorize a representative to apply for services, develop an Individual Rehabilitation Plan, release confidential information, or represent the individual in an appeal.
The designated representative may be an attorney or another authorized person. The applicant must provide the representative's printed name, telephone number, and authorization to act on their behalf. This designation is effective upon delivery to a counselor and continues until the applicant or representative informs the counselor in writing that it is no longer in effect.
This form is part of the Texas HHS's efforts to ensure transparency and compliance with government regulations. As stated in Government Code, Sections 552.021, 552.023, and 559.004, individuals have the right to be informed about information collected by HHS, receive and review that information, and have incorrect information corrected. The Designation of Representative form is an essential tool for individuals seeking rehabilitation services through Texas HHS.
