LDSS-7031. Transmittal for Parent Locator Service Search
Form LDSS-7031, also known as the Transmittal for Parent Locator Service Search, is an official form used to conduct a nationwide search for the location of a parent for the purpose of establishing paternity and/or obtaining child support. It is used by local departments of social services, child support enforcement units, and other child support agencies in New York State.
The form consists of several sections, including sections for the petitioner information, respondent information, supporting documents, and other information. The petitioner and respondent sections require the full name, Social Security Number, date of birth, address, and other contact information for the parent or parents involved in the search. Additionally, the supporting documents section requires the petitioner to provide any relevant documents that may help to locate the respondent, such as birth certificates, marriage certificates, court orders, or previous addresses. The other information section requires additional information about the case, such as the child’s name, date of birth, relationship to the petitioner, and any other details relevant to the search.
It is important to accurately complete the form, as any incorrect or missing information may delay or impede the search process. Additionally, failure to provide all of the necessary supporting documents may also delay the search. The form must be signed and dated by the petitioner.
The form is typically used in cases where the non-custodial parent’s whereabouts are unknown. After the form is completed and submitted, the petitioner will receive a response from the Parent Locator Service (PLS) with the respondent’s current address or a list of possible addresses. This information can then be used to establish paternity and/or obtain child support.
The form is also used in cases involving adoption, visitation rights, and other family law matters. Additionally, it can be used to locate a spouse or other relative.
When completing the form, it is important to provide all necessary documents and information requested. Additionally, any changes in the respondent’s contact information should be reported to the PLS as soon as possible. Other forms related to this form include the LDSS-7030, which is used to submit a request for a paternity or child support order, and the LDSS-7028, which is used to submit a request
