OCFS-4431. Kinship Guardianship Assistance and Nonrecurring Guardianship Expenses Agreement
Form Kinship Guardianship Assistance and Nonrecurring Guardianship Expenses Agreement is a form used to assist kinship guardians and provide them with financial assistance when caring for a child in their home. This form is used by the New York State Office of Children and Family Services (OCFS) to provide financial assistance to kinship guardians to cover the costs of caring for a child who is not related to them. The form is used to provide the kinship guardians with a one-time financial assistance, for nonrecurring expenses, such as the cost of a crib, clothing, bedding, or other items that are needed to care for the child.
The form is completed by the kinship guardian, and requires information such as the guardian's name, address, contact information, and the child's name, age, and relationship to the guardian. The form also requires the guardian to provide information about the expenses that they are requesting assistance with, including the amount of the expense, the date that the expense was incurred, and a description of the item purchased. The form also requires the guardian to provide information about their income and the income of any other individuals living in the home.
The form must be completed accurately and legibly in order to be approved by the OCFS. The OCFS will review the form and determine if the guardian is eligible for assistance. If the guardian is eligible, the OCFS will issue a check to the guardian for the amount of the approved expenses. The funds can only be used for the approved expenses.
The form is used in situations where a child in need of care is placed in the home of a relative or other adult who is not related to the child and who is willing to care for the child. In this situation, the kinship guardian will need to provide for the child's needs, often without any financial assistance. The Kinship Guardianship Assistance and Nonrecurring Guardianship Expenses Agreement form provides the guardian with financial assistance to cover some of the costs associated with caring for the child.
When completing the form, the guardian must provide accurate and complete information. Additional documents may be required, such as proof of income or proof of the expenses incurred. The guardian must also provide information about the child, such as the child's name, age, and relationship to the guardian. In some cases, the guardian may be able to receive additional assistance from other sources, such as the local Department of Social Services or the child's biological parents.
