OCFS-2135. Runaway and Homeless Youth-Interim Family Home Study Report
Form OCFS-2135, commonly known as the Runaway and Homeless Youth Interim Family Home Study Report, is a form used by the New York State Office of Children and Family Services to assess an interim family home provider’s suitability to care for runaway and homeless youth. The form is used to determine whether a family home provider has the qualifications and resources to care for the youth, and whether the home is a safe and suitable place for the youth.
The form is completed by the family home provider and is submitted to the Office of Children and Family Services. The form is divided into sections containing information about the family home provider, the home environment, and the provider’s qualifications and abilities to provide care for the youth. Information such as the provider’s name, address, contact information, and background check results are required. Additionally, the form includes sections that provide information on the home environment such as the physical condition of the home, the safety of the home, and the presence of any other residents in the home.
The form also includes sections for the provider to list any special skills, knowledge, or experience they may have in order to provide care for the youth. Other sections include information on the provider’s ability to meet the youth’s needs, such as their ability to provide a safe and stable environment, their ability to connect the youth to services, and their ability to provide emotional support to the youth.
The form is typically used by family home providers who are interested in providing care for runaway and homeless youth. The form is an important part of the application process and must be filled out accurately and completely. All information provided must be accurate and must be supported by documentation. Inaccurate or incomplete information can lead to delays in the application process or even denial of the application.
In addition to the Runaway and Homeless Youth Interim Family Home Study Report, other documents such as the Family Home Agreement, the Family Home Authorization Form, and the Authorization for Release of Information may need to be completed in order to be considered for this type of care. There are also other forms that may be used for similar purposes, such as the Family Home Assessment Report or the Family Home Assessment Checklist.
