DA Form 200. Transmittal Record

DA Form 200. Transmittal Record

The DA Form 200, also known as the Transmittal Record, is used to document the transfer of items or documents from one party to another within the Department of Army. The form consists of several parts, including a description of the items being transferred, information about the sending and receiving parties, and a certification section. The important fields to consider when filling out this form include the date, description of the items, names and signatures of the sending and receiving parties, and any additional remarks.

This form is used to ensure accountability and proper documentation when items or documents are transferred between different units or individuals within the Department of Army. It helps to track the chain of custody and provides a record of the transfer.

An example of an application for this form would be if a unit needs to transfer sensitive documents to another unit. The unit would fill out the DA Form 200, indicating the date, description of the documents, and the names and signatures of the sending and receiving parties. The form would then be submitted to the appropriate authorities for approval and record-keeping.

An alternative form to the DA Form 200 is the DD Form 1348-1A, Issue Release/Receipt Document. While both forms serve a similar purpose of documenting the transfer of items, the DD Form 1348-1A is specifically used for tracking the movement of material between Department of Defense activities.
